Please complete your final report within 30 days of the completion of your Community Collaboration project. All final report materials for FY24 grants are due by January 1, 2025. To view the questions you will have to answer on the final report form, click the pdf example above. A budget table is required as part of the final report process. To submit your budget table, make a copy of the attached spreadsheet, fill out the required information, and upload your saved spreadsheet to the form or email it to SELCO's Legacy Program Librarian.
Information submitted via the online form is secure. After submitting your final report, you will receive an automatic email confirmation. If you do not receive an email confirmation please contact the Legacy Program Librarian as your report may not have been received.
