Community Collaborations

Final Report

Please complete the final report form within 30 days of the completion of your Community Collaboration project please. A budget table is required as part of the final report process. Make a copy of the attached table, fill out the required information, and upload this document to the form.

  • If your approved grant occurs over the course of two fiscal years (events/purchases happen both before and after July 1st) please check with the Legacy Coordinator before completing the budget table

Information submitted via the online form is secure. After submitting your final report, you will receive an automatic email confirmation. If you do not receive an email confirmation please contact SELCO's Legacy Coordinator as your report may not have been received.
