All grant recipients are required to collect survey results from participants at Community Reads events. Please use these results to complete the final report form and email all required materials to SELCO's Legacy Program Librarian within 30 days of the completion of your Community Reads project. Individual survey responses do not need to be submitted as a part of the final report process.

Information submitted via the online form is secure. After submitting your final report, you will receive an automatic email confirmation. If you do not receive an email confirmation please contact the Legacy Program Librarian as your report may not have been received.
